Union Forms

The following forms and reference documents are commonly used in representation, official time requests, and employee rights matters. If you are unsure which form applies to your situation, you are encouraged to contact a Union representative for general guidance before submitting any documentation.

G-956 – Designation of Representative

Used to formally designate the Union as your representative in matters such as discipline, grievances, or administrative investigations. This form authorizes the release of relevant information to the designated Union representative for representational purposes.

G-955 – Request for Official Time

Used by Union representatives to request official time for approved representational activities in accordance with the Collective Bargaining Agreement.

Form for current NBPC members transferring into Local 2554 from another NBPC local.
