Union Forms

The following forms and reference documents are commonly used in representation, official time requests, and employee rights matters. If you are unsure which form applies to your situation, contact a Union representative before submitting anything.

G-956 – Designation of Representative

Used to formally designate the Union as your representative in matters such as discipline, grievances, or investigations. This form authorizes the release of information to your Union representative so they can effectively assist you.

G-955 – Request for Official Time

Used by Union representatives to request official time for approved representational activities in accordance with the Collective Bargaining Agreement.

Hatch Act – Rights and Responsibilities

Provides general guidance on employee rights and restrictions related to political activities under the Hatch Act. This document helps employees understand what activities are permitted and prohibited.

Office of Special Counsel – Federal Employee Rights

Informational material from the Office of Special Counsel outlining protections for federal employees, including whistleblower rights and prohibited personnel practices.

Kalkines Rights

Explains employee rights and obligations during certain administrative investigations where answers may be compelled under administrative authority. Employees should seek Union guidance immediately if presented with a Kalkines warning or related documentation.
